2008 CYC Registration

 Important Items to note this year for the 2008 CYC Registration are as follows:

Players registered in 2006 and 2007 are not required to re-register in 2008, unless prior registration is unavailable for submission to CYC Registrar or photo id no longer represents a true image/likeness of the player. NOTE: County/Divisional Registrars must be able to provide 2006 or 2007 Registration to CYC Registrar or player will need to re-register.

New Players

Registration Cards!

  1. In addition to Registration Forms, Registrars will be required to deliver to the CYC Registrar, a laminated Player Registration Card for each player in their Division who will participate.
  2. Each Registrar should provide a color copy of the Registration Form to the Clubs/Players in their Division and organize registration dates. Note: All New Registration forwarded to the CYC will be in color.
  3. Prior to registration, players and parents should complete the Registration Form/Cards.
  4. At Registration, Registrars will be required to verify a player by their certified birth certificate. Players should present to the Registrar, an original certified birth certificate, and a copy of the birth certificate they are presenting. The Registrar will retain the copy and certify by signing and dating. The original should then be returned to the registering player. The Registrar should now sign the Registration card. After accepting registrations, Registrars will do the following:
    1. Prepare a copy of the completed Registration .
      1. Original Registration Form (Continental) :

      - Detach a Registration Card from original form, and laminate. (Note: Card should be cut along dotted line, folded in half and then laminated. Acceptable card will measure 3 1/2 in. wide x 2 1/4 in. high.)

      - Detach Registration Form. (Registration Form should be cut on dotted line. Acceptable form will measure 4 3/4 in. wide x 11 in. high. Do not laminate.)

      - Paper clip Registration Card in front of Registration Form for each player.

      - Organize players by age group, then alphabetically.

      - When all players competing in the tournament have been registered in your Division, completed Forms/Cards can be sent to the CYC Registrar, but no later than June 20. Registrations postmarked after this date will be deemed ineligible.

        1. Copy #1 (Divisional Board Copy)

        - Detach the second Registration Card from original form, and laminate. (Note: Card should be cut along dotted line, folded in half and then laminated. Acceptable card will measure 3 1/2 in. wide x 2 1/4 in. high.)

        - Paper clip Registration Card in front of the copy of the Registration Form for each player.

        - Organize players by age group, then alphabetically.

        - These will be retained by the Divisional Registrar. The Divisional Registrar (or another Divisional Officer) will be responsible for providing these copies, should they be requested for comparison by either Tournament Committee. These may also be used by the Division for Local competitions.

        Deadlines

        1. Continental Tournament – ***** IMPORTANT ***** Registration Cards must be received by the CYC, no later than June 20, 2008. Registrations received after this date will not be accepted.

        Transfers

        1. All transferring players must be registered by July 20.
        2. All transferring players must provide a completed copy of the “Transfer Form” at Registration.
          1. Parent or Guardian of transferring player must have Transfer Form signed by their current Club Secretary, prior to registration. Note: Club Secretary will return completed form within five days.
          2. Parent or Guardian of transferring player must notify the County/Divisional Youth Board Secretary, by email or in writing, of their intent to transfer, prior to registration.

          Documentation – Birth Certificates & Passports

          1. The Divisional Registrar will be responsible for maintaining and traveling with the certified copy of each player’s birth certificate presented by the registering player. All non-US born players must present their passports when checking in at the tournament.

          Accurate Completion of Registration Card

          1. County/Divisional Registrar should re-confirm the following:
            1. Correct Division/Club is listed on Registration Card
            2. Date of Birth on Registration Card corresponds with Birth Certificate
            3. Consent for Medical Treatment is completed (YES or NO).
            4. Emergency Contact and Phone Number is completed
          2. Registration Forms should not be accepted without this information.

          Accurate Completion of Registration Form

          1. County/Divisional Registrar should re-confirm the following:
            1. Parents receipt and acceptance of the Code of Best Practice for Youth Sport
            2. Completion of entire form

To be Clear will not accept any registration that is postmarked after June 21st, 2008. Anything received postmarked after the 21st of June will be sent back to you unopened, and that player will not be allowed to play in the tournament. There will be no exceptions to this rule.
